Not a children's book...
You're absolutely right that these themes are not suitable for anyone trying to become an adult. However, ASOIAF is explicitly (heh, pun) adult fiction. It is not intended for an audience of children and teenagers.

I suspect there are teenagers out there who could handle reading it, but I think it would be foolish to market it towards them.

Lastly: I don't know how much of the series you have read, so I will avoid mentioning specific examples, but much (not all) of the bad stuff is directly instrumental to the plot. You could not remove even the squickiest of the problems without serious continuity and suspension-of-disbelief issues. (Example: Stannis's whole reason for...his schtick.)

My younger son (under the influence of his 15 year old brother, another fan) read the first 3 books of the series when he was 9. Wrote a most illuminating piece for his teacher ('What I read during the holidays') on the narrative, including the gay sex and incest. I think the series helped him...grow up...and certainly made teacher/parent meetings a lot more interesting!
